Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: Permian Basin Well Service, Rig 102, Jordan - San Andres unit A-6, Crane County, TX. For the period of time up to and including 03/24/08, the area under and around the horse head was not cleared of personnel prior to connecting the throat bolt. The horse head fell off the walking beam and struck an employee that was readying to put the weight back on the unit. Among others, feasible means of abatement could include: 1. Ensure the throat bolt is installed prior to putting weight on the horse head. 2. Ensure all employees are away from danger areas prior to installing the throat bolt.t
