Text For Citation: 01 Item/Group: 002 Hazard: FALLING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: At Rig #559 north of Cleveland, Arkansas, employees working at the rig were not instructed and familiarized with recognized practices and company safety requirements, including an employee working from the derrick board that was allowed to work approximately 48 feet above the rig floor without fall protection on or about August 13, 2005. Among other methods, feasible and acceptable abatement methods to correct this hazard are: 1) Communicate and enforce the company's Health, Safety, and Environmental Policy that requires supervisors to ensure that all employees are informed of the applicable policies and procedures, receive adequate safety and environmental training to support safe operations, and ensure that all assigned employees utilize the prescribed safety equipment. 2) Comply with the American Petroleum Institute (API) Recommended Practice 54, 1999, paragraph 6.1.6, that personnel receive instruction in correct work methods to reduce chance of injury to themselves or fellow personnel.
