29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: a) MK1 Construction, LLC at 3131 North Osceola Street, Denver, CO 80212: On or about and at times prior to August 14, 2024, the employer did not ensure that each personal fall arrest system (PFAS) was inspected prior to use, and that defective components were removed from service. One of the employees who was utilizing a PFAS while working on the steep-pitch roof of a church fell approximately 30 feet to the ground and died as a result of his frayed lifeline breaking in half.
