29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Location F3; 200 South Progress Parkway: On June 7, 2024, the employer exposed employees to struck-by hazards, in that the employer did not instruct employees in the hazards of being caught between concrete pipes lifted by excavators and the specific measures to take to avoid those hazards as employees guide the pipes into place during pipe laying activities.
