29 CFR 1910.28(b)(7): The employer did not ensure that each employee on a walking-working surface near an opening, including one with a chute attached is protected from falling by the use of one of the systems listed in 29 CFR 1910.28(b)(7)(i)-(iv): On or about May 31, 2024, located on the bailer in the main plant; employees worked near the top of the bailer without fall protection that contained an opening to the chute that was approximately 20 feet above the landing surface while attempting to unclogged the chute.
