(a) 100 Sawmill Road, Belleville, PA - On or about August 25, 2023, the employer had the seat of the Harlo HP8500-4WD-16-85 Powered Industrial Truck serial #102343 replaced with an air-ride seat and did not ensure that the replaced seat was equipped with an seat belt. No abatement or certification required.
