29 CFR 1910.147(d)(4)(i):Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees: a. Production mixing area at the MTC ribbon mixer blender : On or about November 29, 2021, the employer exposed an employee to amputation hazards, in that the employee had not affixed lockout tagout devices while performing cleaning and sanitation of the machine.
