Text For Citation: 01 Item/Group: 002 Hazard:
29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about November 19, 2021, at an establishment located at 4617 South Taylor Drive in Sheboygan, Wisconsin, in the Paint Flake Department; an employee was exposed to machine hazards associated with rotating rollers and a conveyor belt while attempting to remove debris from the line, on the Paint Flake Line B-2. The employer did not ensure that employees implemented energy control application steps prior to cleaning the Paint Flake Line. The machine was not deenergized prior to the employee engaging in the service and/or maintenance activity. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], LOTO device application [1910.147(d)(4)], and verification of isolation [1910.147(d)(6)], were not implemented to protect employees from machine servicing hazards.
