29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: (a) Sawmill building electrical room, during maintenance and cleaning of 480v 3 phase MCC bucket(s): On or prior to 8/20/21, employees did not utilize safety related work practices including, but not limited to: de-energizing and locking out electrical equipment, using appropriate personal protective equipment, and using insulated tools.
