29 CFR 1910.178(l)(3)(ii)(A):The training program content did not include surface conditions where the vehicle will be operated Employees are exposed to struck-by hazards. Employees operating power industrial trucks in tandem to move baghouses were not trained to recognize the hazard presented by the broken and uneven floor exiting the production area. The uneven floor condition caused the load to become unstable resulting in the amputation injury.
