29 CFR 1910.23(c)(4): The employer did not ensure that ladders that are used only on stable and level surfaces unless they are secured or stabilized to prevent accidental displacement. On or about 28 June, 2021, on the north area of the compound, employees utilize aluminum ladders at heights upwards of twenty-five (25) feet with the legs positioned on the unstable grates of a makeshift basket that are hoisted with the forks of a forklift.
