Powered industrial truck operators did not receive initial training in the following topics, specified in paragraphs (l)(3)(i)(A)-(l)(3)(ii)(A) of this section: Location: 120 Central Ave Kearny, NJ 07032 Employee were operating a forklift without having a complete understanding of the operating instructions and precautions listed in the operator's manual and the surface conditions where the vehicle will be operated. The incident occurred on or about 3/30/21.
