29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Trinidad/Benham Corp., at 1500 Right of Way Road, Sterling, CO: On or about 1/18/2021, the employer did not ensure that lockout or tagout devices were affixed to each energy isolating device by authorized employees. Maintenance employees serviced HRH Technologies model 736 packaging machine number 12. The packaging machine had an electrical energy source and a pneumatic energy source. An employee did not place a lockout device on the air valve. This condition exposed the employee to an uncontrolled energy hazard.
