Text For Citation: 01 Item/Group: 002 Hazard:
29 CFR 1910.147(c)(7)(iii)(A): Retraining was not provided for authorized and affected employees when there was a change in their job assignments, a change in machines, equipment or processes that presented a new hazard, or when there was a change in the energy control procedures: (a) Facility, 311 Drum Avenue, Somerset, PA: On or about July 7, 2020 retraining on energy control procedures had not been provided to an employee who was moved into a maintenance position for the bakery lines. The employee had not been instructed on the actual energy control procedure and only received instruction from co-workers on controlling the bakery lines during the cleaning process. The employee had not been instructed on the proper techniques to follow when performing maintenance on equipment on the bakery lines. An employee did not shut down and lock out the bakery line B when performing a maintenance task. The employee got caught in the cone press while trying to install a spring and the injury resulted in the amputation of their left pinky finger.
