29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l). At the worksite located at 11971 Grandview Road, Grandview, Missouri 64030, employees were exposed to serious caught-in, crushing and or struck-by injuries. The employer did not ensure that operators were competent to operate powered industrial trucks safely, by completing a training program which met all training and evaluation requirements of paragraph (l).
