29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): (a)Stretch's Well Service, Inc. at 16266 366th Avenue, Cresbard, SD 57435: On or about and at times prior to November 1, 2018, the employer did not ensure that the workplace was assessed to determine if hazards were present, which would necessitate the use of personal protective equipment. Employees were exposed to, but are not limited to hazards such as; lacerations, puncture wounds, and eye injuries. Employees operate the George E. Failing drill rig and utilize other tools and equipment without the use of personal protective equipment. This condition exposed employees to struck-by and eye hazards associated with these tools and equipment. Abatement Note: The employer shall verify that the required workplace hazard assessment has been performed through a written certification that shall include the following: (1) The workplace evaluated; (2) The person certifying that the evaluation had been performed; (3) The date(s) of the hazard assessment; and, (4) The document identified as a certification of the hazard assessment.
