29 CFR 1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one-half inch: The employer does not ensure industrial floor fans are provided with guards that have openings no greater than one-half inch, exposing employees to amputation hazards. a) This violation was observed on or about June 11, 2018, in the forward wire draw area, where a large industrial box fan had openings in the guarding measured approximately 1 inch by 2.5 inches. b) This violation was observed on or about June 11, 2018, in the forward wire draw area, where a large industrial box fan had openings in the guarding measured approximately 2 inches by 2.5 inches.
