29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) At Adamo Drive and Highway 301 Project, where an employee assisting with removal of equipment such as but not limited to pressurized Cherne plugs being used for a leak test inside pressurized steel casing supply pipeline, without being instructed in the hazards associated with the use of this equipment, on or about February 14, 2018.
