29 CFR 1910.132(a): The employer did not provide protective equipment, including personal protective equipment (PPE), used to comply with this part, at no cost to employees: (a) On or about September 14, 2011, Kline 5300-11-18H well site near Alexander, North Dakota, approximately four employees were exposed to fire and/or explosion hazards on a service rig where the employer failed to provide and ensure the use of flame-retardant clothing (FRC) necessary to protect employees from burns due to potential flash fires resulting in the deaths of two employees and injuring of two others.
