29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: (a) On or about 01/29/2021 - Fabrication Area, authorized employees were exposed to caught-between hazards when performing servicing and maintenance activities such as, but not limited to, adjusting proximity switch sensors on the Trumpf Tru-Laser 5030 Automatic Pallet Changer and the machine had not been shut down as required in the written lockout procedure "Trumpf - Tru-Laser 5030 (A0231A0314)".
