29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: (a) On or about December 6, 2019, in the heavy metal wastewater treatment department, protective equipment for the eyes and face, such as tight-fitting goggles and face shield, were not used to protect an employee from a chemical splash hazard while carrying an open container of sulfuric acid capable of causing severe eye damage and skin burns.
