29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage. This violation occurred on or about October 3, 2019, and at times prior thereto, employees were not required to shut off the main control panel for the chopper machine when unjamming the cyclones exposing employees to struck-by and caught-in hazards.
