29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) Production Floor; On or about August 12, 2019 energy isolating devices were not operated in such a manner as to isolate the Rotoflex die cutter from energy source(s) while an employee worked inside of the machine performing adjustments. (b) Production Floor; Energy isolating devices were not operated in such a manner as to isolate the Rotoflex, CEI, and Mark Andy die cutters from energy source(s) while employees worked inside of the machine performing adjustments.
