29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a) On or about November 5, 2018, the Press Brake Operator of the AccroPress 7608 8 foot 60 Ton Hydraulic Press Brake did not de-energized the Press Brake using lockout/tagout procedures, install the die block or utilize the Press Brake Control System to prevent motion of the Press Brake ram while changing dies.
