29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: On or about November 16, 2020, the employer did not ensure that employees were trained as authorized employees prior to allowing them to engage in servicing and maintenance activities. Maintenance employees were not trained on the recognition of hazardous energy sources and the methods and means necessary to control them. These employees were exposed to machine hazards that could result in laceration and amputation injuries.
