29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: Plastipak Packaging, Inc. - Jackson Center, Ohio: On or about November 5, 2019, employees were exposed to electrical shock hazards in that a Tec conveyor unit (s/n 0701010) was powered without a connected ground wire in the plug connection.
