29 CFR 1910.109(e)(1)(vii): Due precautions were not taken to prevent accidental discharge of electric blasting caps from current induced by other sources of extraneous electricity: a) Hydro Resources, County Road 18 and 69, Keenesburg, CO 80643: On and before April 23, 2019 the employer did not take due precautions to prevent accidental discharge of electric blasting caps from current induced by other sources of extraneous electricity, including static electricity, video cable power, and a wireless surveillance system. Two employees were struck by shrapnel from explosive caps that detonated prematurely. This condition exposed employees to struck-by hazards resulting in multiple shrapnel wounds from the discharges.
