29 CFR 1926.55(b): Feasible administrative or engineering controls were not implemented to reduce employee exposure(s): Worksite: 17660 Wright St. - Omaha, Nebraska - On or about August 17, 2018, employer is failing to ensure that feasible administrative and/or engineering controls reduced employee carbon monoxide exposure levels below the permissible exposure limit. Floor fans and an catalytic converter muffler on the gasoline motor exhaust of the concrete saw were ineffective at reducing carbon monoxide levels. An employee, operating a gasoline powered concrete saw, was exposed to carbon monoxide (CO) during a 60 minute occupational exposure at a mean calculated at 1030.2 parts per million (ppm) CO level. The % carboxyhemoglobin (%COHb) in the employee's blood reached a calculated peak of 40.3%. Atmospheric pressure, height, weight, activity levels, tobacco smoking behavior, and other factors were taken into account. The 8-hour TWA exposure was 128.8 ppm CO, which is 2.58 times the OSHA PEL of 50 ppm CO.
