29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors from sources such as but not limited to open flames; lightning; smoking; cutting and welding; hot surfaces; frictional heat; static, electrical, and mechanical sparks; spontaneous ignition, including heat-producing chemical reactions; and radiant heat: a) Quality Assurance Laboratory: On or about July 3, 2018, an employee added 0.5N alcoholic potassium hydroxide and 0.5N hydrochloric acid to a flammable liquid in a JustRite Safety Can causing an explosion and severely injuring the employee. The facility did not take adequate precautions to prevent static electricity from igniting the flammable vapors. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
