Text For Citation: 01 Item/Group: 001B Hazard:
29 CFR 1910.181(d)(3)(i): Complete inspections of the derrick were not performed at intervals as generally defined in paragraph 1910.181(d)(1)(ii)[b}: The employer if failing to protect employees from struck by and electrical hazards associated with operating derrick without having conducted adequate periodic inspections of the equipment. The most recent occurrence of this was evident on May 23, 2018 during the investigation of a workplace injury 1 Block west of the intersection of Highways 281 and 34 in Grand Island, NE where the employer has crews of two employees operating Pumpsetter truck 9722 to lift a pump from the ground without the equipment having had an adequate periodic inspection prior to having been assigned to this job; allowing the use of a ¼ inch hook and chain assembly instead of the required 5/16 inch hook and chain assembly. The result of which was the ¼ inch hook and chain assembly failed, wrapping the released guy line into the nearby active overhead power line and shocking the operator.
