29 CFR 1910.269(m)(3)(ii): All switches, disconnectors, jumpers, taps, and other means through which known sources of electric energy may be supplied to the particular lines and equipment to be deenergized were not opened and rendered inoperable, when its design permitted. On or about 02/06/2014, at the Cypress Black Bayou Recreation Area, the employer did not clearly and properly identify, isolate then deenergize all electric energy before making repairs to the electrical system at the outdoor recreation area.
