29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) Jobsite located at or near 1901 Vine Street, Philadelphia, Pennsylvania, 19103 - On or about August 13, 2024, an employee was exposed to fall hazards while conducting telecommunication activities as the employer did not ensure that each employee was protected from falling through holes more than 6 feet above lower levels, by personal fall arrest systems, adequate covers, or guardrail systems erected around such holes.
