29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about April 14, 2021, the employer did not ensure the procedure for controlling hazardous energy on a 3M S-867 "L" Clip Applicator taper machine (S/N 4128) on Line #5 was utilized prior to servicing, resulting in the unexpected energizing of the cutting blade during servicing.
