29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment: (a) Trinidad/Benham Corp., at 1500 Right of Way Road, Sterling, CO: On and preceding 1/28/2021, the employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment. Employees operated and performed maintenance upon packaging machines, including but not limited to HRH Technologies model HRH-GB packaging machine (machine number 13). The employer did not assess all hazards associated with the packaging machines. This condition exposed employees to laceration, musculoskeletal disorder, noise, and struck-by hazards.
