29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a) Mixing Room - On or about November 9, 2020, the employer failed to provide training to each authorized employee performing service and/or maintenance to machinery or equipment to ensure the employee acquired the knowledge and skills required for the safe application, usage and removal of energy control devices. An employee performing the duties of a "mixer" sustained an amputation of the right hand middle finger tip while attempting to clear a jam on the Hobart meat grinder, Model 4156, Serial Number 11-1004-142.
