29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. On 5/20/2020, at the Murrayville facility, in the Mechanical Debone Department, an Authorized employee did not utilize proper lockout to the floor vacuum valve mechanism and incurred a thumb tip amputation. The employee had not been trained in the specific written procedures for the floor vacuum.
