29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) This violation occurred on or about June 1, 2019 and at times, prior thereto, specific written lockout/tagout procedures were not documented for the control of potentially hazardous energy when performing servicing and/or maintenance activities on the chopper machine and its components exposing employees to caught-in and struck by hazards. b) This violation occurred on or about October 3, 2019, specific written lockout/tagout procedures were not utilized for the control of potentially hazardous energy when performing unjamming of the chopper cyclone exposing employees to caught-in and struck by hazards.
