Text For Citation: 01 Item/Group: 001 Hazard: STRUCK BY
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm in that his employees were exposed to struck-by hazards: a.On or about November 27, 2004, at the Flex-3 Rig #229 jobsite, an employee used a stabbing board which was not appropriately extended and was adjacent to moving machinery. The employee accessed his work position without the benefit of a connecting walkway to the derrick ladder and was exposed to the potential for being struck by the draw-works. Among other methods, feasible and acceptable means to correct this hazard are: a.Ensure employees do not remain at the stabbing board position during the operation of the derrick's draw-works unless guards and connecting ladder-access walkways are in place. b.Ensure the stabbing board is fully deployed/extended and the employee has ample space to remove himself from the hazard of being struck by the adjacent draw- works. c.Provide training of suitable signaling or other anticipated forms of communication between the casing crew and the Drilling Rig Operator so that the drawworks are not engaged while employees are in the path of moving equipment.
