SCRR 71-112A: Failed to furnish a place of employment which is free of recognized hazards which may cause death or serious physical harm to his employees and comply with this regulation and other occupational safety and health rules and regulations promulgated under Chapter 15 of Title 41, Code of Laws, State of South Carolina, 1976, as amended, as follows: A. Employer knew or should have known that employees working in a personnel basket being hoisted by a drum type hoist with no trial lift being conducted were exposed to the hazard of falling up to and in excess of 1200 feet to the ground, on or about June 24, 1998. A feasible and useful method, among others, is to perform a trial lift prior to having employees work from the basket, to detect and/or prevent system failure due to overload. B. Employer knew or should have known that employees working in a personnel basket being hoisted by a drum type hoist without a complete visual inspection were exposed to the hazard of falling up to and in excess of 1200 feet to the ground, on or about June 24, 1998. A feasible and useful method, among others, is to perform a complete visual inspection of the basket and cable after the trial lift and prior to having employees work from the basket. C. Employer knew or should have known that employees working in a personnel basket being hoisted by a drum type hoist, on or about June 24, 1998, with not prelift meetings to discuss procedures, limitations, and precautions were exposed to the hazards of falls while using improper procedures, overloading, and/or not taking proper precautions. A useful and feasible method, among others, is to have prelift meetings to discuss all pertinent safety items prior to lifting. D. Employer knew or should have known that employees working in a personnel basket with no grabrail being hoisted by a drum type hoist were exposed to the hazard of falling out of basket that was up to and in excess of 1200 feet, on or about June 24, 1998. A feasible and useful method, among others, is to install a grabrail inside the personnel basket. E. Employer knew or should have known that employees working in a personnel basket being hoisted by a drum type hoist with no anti-two block system were exposed to the hazard of falls up to and in excess of 1200 feet, on or about June 24, 1998. A feasible and useful method, among others, is to install an anti-two blocking type device or hoisting deactivation system to the equipment.
