Text For Citation: 01 Item/Group: 001 Hazard: EXPLOSION
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to an explosion/flash fire hazard: a) Mantua Grove Road, Woodbury, NJ 08096 - On or about 11/29/97, Huntsman Polypropylene Corporation did not ensure that a Vactor Mobile Industrial Vacuum Loading Machine (manufactured by Peabody Meyers), was grounded and/or bonded. The equipment was operated by Allstate Power Vac (an outside contractor). NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is to require in the safe work permit process that contractors electrically ground and/or bonded all equipment that may cause the buildup of a static charge. b) Mantua Grove Road, Woodbury, NJ 08096 - On or about 11/29/97, Huntsman Polypropylene Corporation failed to observe the work practices and equipment of an outside contractor, Allstate Power Vac, prior to the start of and during vacuuming procedures. NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is for Huntsman Polypropylene Corporation to follow the procedures located in Section 3.5, of its SP307 Safe Work Practices manual. ABATEMENT NOTE: In accordance with 29 CFR 1903.19, Abatement Verification, the employer will furnish certification that the described hazard has been abated. Documentation may include but is not limited to photographs, videos, work orders, invoices, training records and consultant reports. Abatement documentation must be received by the Area Director within 13 calendar days of the final abatement date for this violation.
