Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: a) The Signal Group, Inc. dba Advance Cylinder Repair, Inc. - Ashland, Ohio: On or about January 27, 2012, the employer failed to implement procedures that would prevent employees from being exposed to hazards of pressurized gas when repairing a hydraulic piston-type accumulator. Employees who repair cylinders were exposed to hazards in that the employer did not take measures to assure that gas charged cylinders were not serviced by employees or that stored energy was relieved prior to repair work. An employee received fatal struck-by injuries during repair work on a hydraulic piston-type accumulator that was charged with nitrogen gas.
