29 CFR 1910.28(b)(3)(i): The employer did not ensure each employee was protected from falling through any hole that was 4 feet (1.2 m) or more above a lower level. Site: 31-20 Northern Blvd, Chamber B, Long Island City, NY 11101. a.) On or about October 07, 2024, employees engaged in sewage cleaning were standing less than one foot away from a manhole opening measuring 36-inches in diameter above the siphon without fall protection. The employees were exposed to a fall hazard of 120-feet to the siphon area where work was being performed. NOTE: ABATEMENT DOCUMENTATION IS REQUIRED FOR THIS ITEM PURSUANT TO 29 CFR 1903.19(d ).
