OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushed-by hazards while moving materials with a CAT 906H Loader Location: Troupe Waste Services 1477 Bedford Road, Abington, MA On October 10, 2023, an employee was crushed while unloading a stack of steel container bottoms from a flatbed trailer using a CAT 906H Loader equipped with 44-inch forks where the center of gravity of the load was beyond the end of the forks. The employee climbed on the unstable load in an attempt to stabilize it and was riding the load as it was being lowered to the ground. The load tipped forward off the forks crushing the employee.
