29 CFR 1910.333(b)(2)(ii)(A): Safe procedures for deenergizing circuits and equipment were not determined before circuits or equipment are deenergized. On or about August 14, 2023, safe procedures for deenergizing all electrical energy sources located in the outdoor tank farm metal raceway were not determined prior to employees working to replace/reinstall the conductors. The Active Group Lockout #30 "Tank Farm" procedures did not identify and address the 'Truck Loadout Pump' circuit, energized through circuit breaker #17 in the MSP-3 panel located in the DDE MCC room.
