On a 12:12 pitch roof at a new residence, at a temporary worksite located at 13762 Mandarin Road, in Jacksonville, FL: On or about March 15, 2023, the employer, exposed employees to fall hazard, in that fall protection was not utilized by employees while performing roofing operations.
