29 CFR 1910.333(a)(2): Where exposed live parts were not deenergized (i.e., for reasons of increased or additional hazards or infeasibility), other safety-related work practices were not used to protect employees who could be exposed to the electrical hazards involved:(a) (LOCATION) (IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS) On or about 01/25/23, at 226 Ocean Drive, Miami Beach, Florida, an employee was exposed to electrical hazards when troubleshooting an elevator's electrical panel and the employer's work practices did not prevent contact with energized electrical parts.
