29 CFR 1904.39(a)(1): The employer did not report within 8-hours the death of an employee resulting from a work-related incident. a) 401 East Hunting Park Avenue, Philadelphia, PA: On September 10, 2022, an employee working as an Emergency Medical Technician suffered fatal injuries after being crushed between two motor vehicles. The employer reported the fatality on September 12, 2022, more than 8 hours after they became aware of the death.
