29 CFR 1904.39(a)(1): Within eight (8) hours after the death of any worker as a result of a work-related incident, the employer did not report the fatality to the Occupational Safety and Health Administration (OSHA), U.S. Department of Labor. On or about August 10, 2022, a worker suffered injuries that resulted in a fatality. The fatality was not reported within 8 hours to the local Area Office of the Occupational Safety and Health Administration. The Austin Area Office was notified on August 11, 2022 at approximately 5:26PM.
