29 CFR 1910.179(n)(2)(ii) The load shall be attached to the load block hook by means of slings or other approved devices. (a) On or about February 19, 2022, employee(s) were exposed to crushing hazard while removing a steel plate fixture from a rock crusher, which was being fitted and welded together. The employer had not developed and implemented written procedures to ensure the load (steel plate) was secured by the overhead crane prior to the steel plates removal from the rock crusher.
