29 CFR 1910.28(b)(1)(i)(C): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected by personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems: (a) Environmental Abatement, Inc, Building 220, Hill Air Force base, UT 84056: On or about February 13, 2022, an employee was working in the attic of the building vacuuming metal dust. In order to complete this task the employee had to unhook his lanyard from the lifeline in order to walk on the C-Channel. The employee fell through the ceiling approximately 23 feet to the concrete floor below and sustained serious injuries. This condition exposed employees to fall hazards of approximately 23 feet.
